Case 1
A 46-year-old woman was admitted with complaints of severe dry-eye symptoms, blurred and double vision, halos around objects, and inability to look at oncoming car lights. She had had excimer laser surgery 2 years earlier for myopia, which she said was around −10.00 diopters (D) and −11.50 D in the right and left eyes, respectively. The surgeon explained that emmetropia might not be achievable but she would be able to see without spectacles. Laser in situ keratomileusis (LASIK) was performed without apparent complications.

After the procedure, the patient was able to see more clearly but her complaints about daily life activities did not decrease. The eye was itchy and dry with a burning sensation, even with frequent application of artificial tears. The patient had been on alprazolam and sertraline, alternately, for several years preoperatively. The ophthalmic examination revealed a best corrected visual acuity (BCVA) of 0.8 in both eyes with −0.75 ? 180 in the right eye and −0.75 D in the left eye. On biomicroscopic examination, the corneas had inferior punctate epithelial erosions and the tear breakup time (BUT) with fluorescein was extremely short, 2 to 3 seconds. Under high illumination, the pupils appeared to be mid-dilated, with diameters of 4.5 mm and 5.0 mm in the right and left eyes, respectively. Under mesopic conditions, the pupil diameters were larger, about 5.5 mm and 6.5 mm, respectively. Schirmer measurements after 5 minutes were 1.0 to 2.0 mm in both eyes. For the fundus examination, mydriatic drops were instilled. Rapid pupil dilation was observed in 6 to 7 minutes, and the fundus examination showed peripheral lattice degenerations with argon laser scars in both eyes.

Severe dry eye and visual disturbances due to a mid-dilated pupil were diagnosed. The prescription included frequent instillation of artificial tears and pilocarpine drops 4 times daily. Punctum plugs were also advised, but the patient did not want another ocular procedure. She called the office 4 days later from a psychiatry clinic where she was hospitalized because of intolerable visual disturbances resulting from longstanding pupil dilation. The patient did not return for a follow-up visit.

A 34-year-old woman reported dryness of the eyes and visual disturbances after LASIK 11 months earlier. According to her statements, the refraction in her spectacles was approximately −8.00 D and −7.00 D. She was receiving antidepressant therapy with seroxate; one reason she had LASIK was to improve her mood and increase her self-confidence. She reported that the result of the surgery was a ?catastrophe,? ?worse than anything I could imagine,? and that ?life became a burden? for her.

The ophthalmic examination showed a BCVA of 1.0 and 0.8 in the right eye and left eye, respectively, and corresponding refractions of −0.75 D and −1.00 D, respectively. The corneas were clear. The measurements of BUT with fluorescein were 1 to 2 seconds in both eyes and of the Schirmer test, almost zero. The pupil diameters were about 4.0 mm in both eyes even in bright-light conditions. The pupil diameters under mesopic light conditions were 5.5 to 6.0 mm bilaterally. Fundus examination was done without instillation of mydriatic drops. No fundus pathology was seen in the observable quadrants. The patient accepted a punctum plug procedure, and frequent artificial tears were prescribed. Pilocarpine drops 4 times daily were also advised. The patient was not satisfied with the result of treatment attempts, although the pupil diameter was smaller, 2.5 to 3.0 mm, during follow-up visits.
